This is a 14"x8.5" receipt, although it's folded in half so only the top half was photographed.
It's the monthly bill for the William Byrd Hospital for August 1908, which came out to
$64.36, although that included a balance of $38.58 at the start of the month.

This receipt is from 1907. It says:

It shows all the purchases for the month of September. It doesn't say who the purchaser was, but it may have been a doctor or hospital, based on the amount of medicine & Gauze that was purchased. For instance, on Sept 9, they purchased 100 yds Gauze, 2 pounds Green soap, and 1 pound of C.Acid.
