Company Names, addresses, dates:

Hazel's Dairy, Westmorland St near 19th Rd, Falls Church, VA (1936-1941) 1,2

Notes:
William Andrew Hazel (1879-1950)3 started the Hazel Dairy in July 1931, with his 2 sons, Thomas Lloyd Hazel (1907-1977)4 and Richard Edward Hazel (1914-2002)5. They sold only 13 quarts of milk during their first day of business, and grew from there.1

At some point, Thomas was the front man for the dairy, as one of the bottles below says "THOS L. HAZEL" instead of "HAZEL'S DAIRY".

Effective October 1st, 1935, the Hazel Dairy took over business of the Wild Wood Farms Dairy in Herndon.6

In May of 1936, Hazel's dairy had been selling milk for 11 cents a quart, but was forced to increase their price to 13 cents in Arlington & Fairfax counties by a court ruling that said the State Milk commission could enforce price setting for milk7.

In October of 1936, the Hazel Dairy took over the business of the Falls Church Dairy, their competitor in Falls Church that was run by M.S. Good. They moved their operation from Sleepy Hollow Road to the location formerly occupied by the Falls Church Dairy on Westmorland Street, 1 block south of route 29 near 19th Road. They retained the employees of the Falls Church Dairy.1

The Hazel Dairy was sold to the Alexandria Dairy Products Co in 1941.8
This snippet from a 1943 Franklin Survey Company map shows the location of the dairy, after it had been sold to the Alexandria Dairy Company:
